I Got Mine 5" From 4wp, They Are Made By Superlift. I Think They Most Commonly Are Installed And 99-06 1/2 Ton 4wd 6" Superlift Kits

Summit Racing carries all the superlift block and u-bolt kit stuff...

I got my Skyjacker 2" Tapered cast iron blocks from Summit Racing, came with a u-bolt kit also.
