The part number is 92-0786-3005.
It's similar to a LockRight. It is a little more difficult to argue with (pretty much have to have a hook tool to deal with the springs) but you can still put one in a vehicle in about an hour.
I had it in a 5.3L Silverado. It should fit any of the GMT800/GMT900 2wd/4wd Silverado/Sierra/Tahoe/Yukon/Escalade. I believe the AWD GMT900s have a different carrier to go with the different pinion but I'm not sure.

$250. I get a decent deal with UPS at work for shipping.
