Your generation of GM truck benefits greatly from any type of header because the stock manifolds are very restrictive. If I were you, I would at least look into a header install. If you are working on a budget, I would simply replace the muffler if it has internal rot issues.

Headers back will give you the best gains period.

but teh Y-pipe on your truck is very restrictive. if you have the money, headers back is definitely your best option.
if you are just wanting a cheap fix, replace the muffler

GM never factory installed headers on any truck. The bigblock manifold may look moreso like a header but headers they are not.
A nice set of longtube headers and 2 1/2" compression bend(crimp bent) pipe into a cat or 2 will work nicely with any engine mods you do. Or go 2 1/2" pipe off teh headers into a single 3" exhaust would be just as good or better es[ for low-mid rpm torque.
Hooker Comp 2462-hkr(painted) or 2462 1hkr(cermic coated) are nice headers and come with the O2 sensor bung you need for your truck. There are ots of headers out there. Going with longtubes foreces you to drop the stock restrictive y pipe.
