I'm having the same problem, same year, extended cab as well, I have replaced the control head and still doesn't seem to change where the air flows no matter what. Anyone know what to look for?

Mostly likely the heater door control motor. Look just above and to the driver's side of the center floor vent. There is a small electric servo motor that changes the position of where the air blows (level). Unscrew the 3 bolts and remove the motor, manipulate the door by hand and see if you can get the air position to change. If so, the motor is probably weak. If the door feels stuck or hard to move you have something off track or gumming it up inside. Look at the linkage that has the little spring attached to it and follow it way up to another motor. Check that one out if the first one didn't fix it.

I am having similar issues but one of the "flaps" inside the box has come off track and will not direct the air anymore. How hard will it be to pull this box and take a look inside?
