For the lift kit shipping will be expensive. I'm assuming between $70-$100.

If you have a BL, then you need a spacer with the K&N. Without a BL, then a K&N is good.
This is the way I have mine.

The K&N pops off if you do a half-ass install. You have to take your time, make sure everything is tight, but not too tight, lined up, etc...

I doubt there's more 500 miles on the filter. I haven't got picture yet but I'm going to try and take some this evening. The K&N comes with everything you need to install it. The kit is basically brand new, it was used for less than a month.

With a three inch body lift your going to have to come up with another 2 inches. I know there isn't probably over an inch of flex in those FIPK at least not on mine and the dealer installed it.

I looked at my friends truck and he has a K&N FIPK with the throttle body spacer installed on it. He has a 3 inch body lift. So it will work if you run the K&N with the throttle body spacer. But I don't think it will work with just the K&N.
