we put a box liner in our New D-Maxx 2500HD and now since gas prices are high we want to put a goose ball in the box so we can leave the 454 vortec dually at home when travelling long pull a 4 horse trailer aluminum living quarter trailer and would it be able to run 18s with 33 Toyo M/T s on Demoda Concept IntimidatorII blk+chrome

You should be able to install a gooseneck hitch into your truck no problem with a spray in bedliner.

Trailer/hitch dealer here. Turnover ball hitch not a problem in truck with bedliner/drop or spray. Mileage won't be 18 though...

Haul gooseneck all over with 4" Rancho and 35" tires. Only problem I see is to make sure your wheels are rated for at least 3100 lbs. each, 3400 is better. Seen lots of 18"+ wheels rated for only 2100 lbs... even 8 lug wheels.
